The document concludes that the agency missed a sequence of critical opportunities to neutralize the threat posed by 20-year-old Thomas Matthew Crooks, who ultimately wounded the politician in the ear before being eliminated at the scene.
According to the report, the security service completely missed the fact that Crooks was conducting preliminary surveillance of the area using a drone—a lapse attributed to the poor training of a staff member and an equipment malfunction. Furthermore, security leadership failed to establish a joint operations center with local law enforcement, preventing the Secret Service from receiving timely citizen reports about Crooks's suspicious behavior. The agency also lacked sufficient personnel on-site to secure and restrict access to the roof of the building from which the shots were fired. Despite this security failure, Donald Trump went on to win the election in November 2024, securing his second term in the White House.
